Improved integration of extension point documentation into doxygen.

The generated html documentation from the .exsd scheam files is now
included via \htmlinclude into doxygen subpages. The original doxygen.css
stylesheet has been extended by copying the schema.css content into it
to provide proper styling for the extension-point subpages.
