MITK Forms

Summary:

• Add initial version of MitkForms module
• Add initial version of MitkFormsUI module
• Add Forms Example plugin
• Fix flat buttons and box-shaped frames in dark style
• Add Form field to SegmentationTaskList
• Add documentation

The MIC Rocket team would like to have this feature by the end of next week, hence I put everything already done up for review. The feature itself is pretty much finished. The integration into the Segmentation Task List and the user documentation / file format specification are missing.

Fixes T30497.

Test Plan: Tested manually with the Forms Example plugin.
