There seems to be some confusion going on in the Otsu tool in groups != 0.
When creating a preview, the generated label names usually follow the naming "Otsu 1", "Otsu 2" and so on. But when multiple groups are present, the name "Otsu 1" is repeated as many times as the current group's number. So when (a label in) group 4 is active and one creates a preview for 4 regions, all labels are called "Otsu 1" in the preview.
When pressing "Confirm Segmentation", these previewed names are not actually used, though. It seems that internally, the labels are actually called "Otsu 1", "Label 1", "Label 2" and so on, so this is what they are called after confirming (if one of those names already exists, a new instance is added to that label).
