A mode for the LevelWindow according to some siemens machines, where panning an
image is done by clicking on it (in the middle of the image), and scale of an
image with the same mouse button but clicking on the exterior widget regions.
->This would provide one "free" mouse button for level windowing.

This looks like a nice feature but a quite dramatic change in how MITK interaction works. Maybe this could be done optionally? Are there any reasons why we should already have it for 1-0?

No, I think this is a pure feature request which can have a lower priority than other bugs for 1.0 release.
Features like this should become possible with the planned extension to the general interaction framework. Won't be fixed here.
